Hypothesis Central venous blood gas (VBG) measurements of pH, P CO 2, and base excess can be substituted for the same values obtained from an arterial blood gas (ABG) analysis in mechanically ventilated trauma patients, obviating the need for arterial puncture.
Base excess: The base excess is the concentration of titratable base when the blood is titrated with a strong acid or base to a plasma pH of 7.40. The value provided is actually the standard base excess, which is an in vivo expression of the amount of base present. It is a calculated value using a very complicated formula.
SID means Strong Ion Difference (SIDa and SIDe for SID apparent or effective). SIG is the Strong Ion Gap. BE is the Base excess (SBE for Standar Base Excess). Note that much of the utility of VBG and ABG analysis is to gather serial readings to determine response to treatment. Regression analysis showed that VBG base excess value is a strong predictor of the ABG base excess value. In our study however, as illustrated with the Bland Altman plot, as patients’ base excess became more negative, the relationship became less reliable. ABG and VBG base excess seems not correlate well in our sickest patients.

VBG is less invasive and less painful than ABG and evidence shows there is good concordance between venous and arterial values for pH, bicarbonate ion concentration, base excess, and lactate. Venous blood gas (VBG) interpretation . Arterial blood gases (ABGs) are commonly used for estimating the acid-base status, oxygenation and carbon dioxide concentration of unwell patients. However, arterial blood can be difficult to obtain due to weak pulses or patient movement.

If you are concerned about the patient’s metabolic acid-base status, a VBG will give you a pH, HCO3, lactate and BD that closely approximates the ABG. A venous pCO 2 < 45 mmHg will reliably screen for hypercarbia on a VBG, but the actual value may vary from that of an ABG with an average difference ranging from 5.7- 8.6mmHg.
Therefore, ABG samples should be obtained in acutely ill trauma patients if accurate acid-base status is required. VBG may eventually take the place of ABG analysis in determining acid–base status.
The ‘bedside rules’ (see our video tutorial) have not been validated for VBGs, therefore, at the present time, VBGs have no role in the assessment of mixed acid-base disturbances. Elevated venous lactate levels show no relationship with the arterial lactate. Base excess: –2mEq/L to 2mEq/L (.ie. from minus 2 to positive 2 mEq/L) Note: the venous blood gas (VBG) values are usually a little different from the ABG values. In the VBG, the – pH is about 0.03 to 0.04 pH units lower than in the ABG, – [HCO3 –] is about 1 to 2 mEq/L higher, – PCO2 is about 3 to 8 mmHg (0.4 to 1.1 kPa) higher Generella tolkningsguider för blodgastolkning.
